**Job Description:**
We are seeking a highly skilled and experienced clinician to join our multidisciplinary team. As a Senior Clinician, you will be responsible for providing high-quality specialist drug and alcohol services to support the transition of consumers from hospital to community.
**Key Responsibilities:*
* Review historic and current substance use issues in the context of consumer transition
* Undertake substance use assessments where required
* Identify risks to the transition and design mitigating strategies
* Provide coaching, consultation, and support to the team and stakeholders on substance use issues
* Deliver brief interventions, both individual and group
* Develop linkages and referral pathways across community-based support services
**Requirements:**
* Must hold a qualification in Occupational Therapy, Psychology, Nursing, Social Work, Counsellor (Degree in Bachelor of Health Science Mental Health, Djirruwang Program) which provides eligibility for membership of the relevant professional association and/or provisional or general registration through the Australian Health Practitioner Regulation Agency (AHPRA)
* Eligibility to drive in NSW and willingness to drive long distances for work purposes
