About the Role
Join the Dandenong Continuing Care Team as a Grade 3 Social Worker in Adult Mental Health, delivering expert psychosocial care to consumers with complex needs. In this senior role, you will provide advanced assessment, intervention, and advocacy, working closely with families, carers, and a multidisciplinary team. You will coordinate care, support recovery-focused practice, manage risk, and deliver family-sensitive interventions while contributing your social work expertise to enhance team capability and community mental health outcomes.

About Us
DCCT is part of the Monash Adult Mental Health Program, providing comprehensive mental health services through a collaborative team approach. Our team consists of case managers, registered psychiatric nurses, occupational therapists, social workers, a consultant psychiatrist, medical officers, psychologists, lived experience workers and an administration team.
About You
You are an experienced Senior Social Worker with a strong background in community mental health and a commitment to client-centred care. You bring expertise in comprehensive psychosocial assessment, delivering therapeutic and psychoeducational group programs, and providing training and mentorship to staff and stakeholders. As a collaborative leader, you excel at supporting multidisciplinary teams, fostering professional growth, and promoting recovery-oriented, evidence-based interventions. You are skilled in risk assessment and management, advocating for clients, and contributing to high-quality service delivery that improves outcomes for individuals, families, and the community.
