About the role
The Diabetes Centre at GV Health is looking for a Senior Social Worker (Grade 3) to join the team on a part‐time basis, two days per week. This new role will strengthen the multidisciplinary capacity of the service for clients with diabetes, focusing on the optimal health and wellbeing of people diagnosed with Diabetes Mellitus, including those with complications.
The Social Worker will provide comprehensive services to both paediatric and adult clients. Duties include psychosocial assessments, client advocacy, carer support, education, counselling, and referral to other agencies or assistance in meeting identified needs. The role has a strong emphasis on continuous improvement, evaluating the quality and outcomes of social work interventions.
Please Note: This position can be combined with the role currently advertised "Senior Social Worker with the Victorian Paediatric Rehabilitation Service" (1 day per week). If combined, the successful candidate would work three days per week across the two programs.
Key responsibilities
* Conduct psychosocial assessments and develop client plans.
* Advocate for clients and provide support to carers.
* Deliver education and counselling on diabetes self‐management.
* Refer clients to appropriate agencies and ensure seamless service linkage.
* Monitor, evaluate, and report on the effectiveness of interventions.
Key selection criteria
* Approved qualification in social work or related discipline.
* Experience working with diabetes patients or in a multidisciplinary team.
* Strong assessment, advocacy, and counselling skills.
* Ability to work independently and collaboratively.
* Good organisational and time‐management ability.
