Due to growth in our Community Services Home Care, we are seeking an experienced Case Care Manager –Bene Casa to join our team, supporting and partnering with older people to live independently and safely in their own homes.

or something similar that makes it clear the budget management is part of the new world Coordinate clinical and non-clinical services to ensure safe, timely care Apply principles of wellbeing, reablement and restorative care Conduct risk assessments and in-home safety checks Maintain accurate, compliant client documentation Build strong relationships with clients, families and care teams Participate in a shared on-call roster with other Case Care Managers, with team support and clear escalation pathways Undertake a consultative, relationship-focused approach, clearly articulating client needs to internal and external service providers Match services to approved plans and budgets and actively facilitate the right supports to achieve the best outcomes for each client About You Registered Nurse (AHPRA) or relevant degree (Social Work, Allied Health) Demonstrated experience in community or aged care management Strong clinical assessment, care coordination, and communication skills Confident in managing a diverse caseload and working autonomously Ability to advocate effectively for clients and negotiate appropriate services Commitment to person-centred, culturally inclusive care Driver's License and current National Police Check Bonus: Experience with SAH/CHSP, Italian-speaking, regulatory experience, or NDIS Our Vision
