Overview
This permanent full-time position provides a high-quality Social Work service to patients, clients, consumers and their family/carers within the Integrated Care Team.
Responsibilities
- Provide a high quality Social Work service to patients, clients, consumers and their family/carers
- Ensure the provision of optimal health outcomes consistent with FWLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct
- Work as part of the Enhanced Care Team within the Integrated Care Team
Qualifications and Experience
- A bachelor degree in Social Work which provides eligibility for membership of the Australian Association of Social Workers and a minimum of 3 years post graduate experience in a relevant field. Knowledge, skills and experience to meet the requirements of the NSW Health Professionals Award Level 2.
- Demonstrated ability to work in a health related field and to provide a range of Social Work interventions (eg psychosocial assessment, case management, group work, advocacy and education)
- Experience with Bereavement, Grief and Loss in a Palliative Care setting and understanding of current grief and loss theory
- Ability to work with consumers, families and carers in times of high stress
- Extensive knowledge, clinical proficiency and ability to work with minimal supervision within a multidisciplinary team
- Ability to prioritise and organise own work/caseload
- Commitment to professional development, quality improvement and evidence-based practice

Remuneration and Conditions
- Remuneration: $91,472 - $116,038 + Superannuation
- Hours per week: 38

Incentives
This position qualifies for an annual Recruitment Incentive Package up to the value of $10,000 (conditions apply and may be pro-rata’d to contracted hours). Payments are made fortnightly under the NSW Health Rural Health Workforce Incentive Scheme.
