Job Description:

We are seeking a skilled clinician to join our team in delivering high-quality, client-centred care to parents navigating substance use and complex psychosocial challenges.

The successful candidate will conduct comprehensive assessments that explore factors such as attachment style, parenting capacity, mental health, and general wellbeing. They will collaborate closely with the Clinical Nurse Consultant and a supportive multidisciplinary team to develop case formulations and care plans tailored to individual needs.

This role involves coordinating outpatient appointments, delivering assertive outreach through home visiting services, and providing clinical advice to families and carers. The clinician will also ensure all care aligns with professional standards and evidence-based practice.

About the Role:

* Conduct comprehensive assessments of clients' needs and circumstances.
* Develop and implement individualized care plans in collaboration with the Clinical Nurse Consultant and multidisciplinary team.
* Provide clinical advice and support to families and carers.
* Cordinate outpatient appointments and deliver assertive outreach through home visiting services.

Requirements:

* Degree in Social Work, Psychology, Counselling or equivalent qualification.
* Eligibility for full registration with the Australian Health Practitioner Regulation Agency (AHPRA).
* Ability to drive in NSW and willingness to travel for work purposes.
