About the Role
1. To work effectively as part of an interdisciplinary team ensuring seamless referral processes between services while minimizing duplication and enhancing service user experience. This involves providing initial assessments which are then used by other professionals within these partner agencies. You will also develop strong relationships with colleagues from Police Forces, HM Courts & Tribunals Service (HMCTS) along Third Sector Organisations such as drug rehabilitation centres, to ensure referrals reach those who require support quickly. These partnerships demonstrate how Liaison & Diversion can contribute positively towards improving life outcomes for vulnerable individuals facing significant challenges both inside custody or out following release. We need someone passionate about social welfare policy whose primary goal is reducing reoffending through early intervention strategies tailored specifically around each individual's unique circumstances making sure everyone gets access necessary care before things escalate further still potentially saving valuable resources across multiple sectors later down line.
