About your role: * The CN will be required to have expertise in specific areas of geriatric nursing and may be required to work across all streams of the Older Persons Integrated Health Service which includes ED, community and residential aged care. * Reports clinically to the Older Persons Integrated Health Service clinical nurse consultant and is managed operationally by the Program Manager. * Advanced gerontic screening, assessment and care of RACF residents and community-based clients. * Coordinating care in collaboration with treating teams and community health. * Demonstrates advanced planning and coordination in the clinical management of patient care, identifying, anticipating and prioritising needs and working with the interdisciplinary team in all settings to achieve patient and unit goals within required timeframes.
