We are seeking an experienced and compassionate health professional to take on this newly created position of Complex Discharge Coordinator to join our BRHS and play a pivotal role in improving patient outcomes and care transitions. Contract: Ongoing, Full-Time with Monthly Accrued Days Off (ADO)
About the role:
In this role you will play an essential role in facilitating safe, timely, and coordinated discharges for patients with complex care needs. This position works in close collaboration with multidisciplinary teams, patients, families, and external service providers to ensure continuity of care beyond the hospital setting. It focuses on identifying discharge barriers early and developing tailored plans to support effective transitions.
This is a key leadership role enhancing patient flow, reducing hospital length of stay and preventing avoidable readmissions. It also supports system-level improvements through the design and implementation of innovative care pathways.

Key Criteria required:
* Registered Nurse, Allied Health Professional, or relevant health qualification with current registration (if applicable).
* Minimum 5 years' clinical experience
* Experience and knowledge in discharge planning, care coordination or case management within hospital or community health settings.
* Strong communication and interpersonal skills to collaborate effectively with patients, Proven problem-solving skills with a proactive approach to overcoming barriers to safe discharge.
