About This Role
You'll deliver care across various hospital services, working within multi-disciplinary teams to expand your clinical pharmacy skills. On-the-job training will help you develop expertise in this area.
* Conduct medication history interviews and reconciliation at transfers of care. Disperse medications efficiently while monitoring their accuracy.
* Provide education and counseling to patients on medicines use.
