About the role: Reporting to the Head of Unit & Nursing Director, the Diabetes Nurse Consultant is an expert clinician accepting accountability for diabetes education, providing a clinical consultative service within Medicine, Cardiac & Critical Care Services (MCCC) and to other agencies as required and for addressing inconsistencies between practice and policy. Within this context, the services that are provided by the Diabetes Nurse Consultant will contribute to the achievement of the goals and objectives of the SALHN MCCC. Duties for the role will include: Providing direct expert nursing/midwifery care for clients with Type 1 and Type 2 Diabetes, Providing clinical leadership to nurses (level 2 Diabetes Educators) as well as allied health professionals, peers and clients, Providing coordination and leadership of education and support, that contribute clinical expertise to improve patient outcomes across the tertiary and intermediate care continuum, Providing clinical expertise to support health services with regard to timely patient flow creating capacity within metropolitan hospitals, Effective complex discharge planning / hospital avoidance through the provision of education, equipment and referral About you: Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate, Must be a Credentialed Diabetes Educator or be in process of completing the university course, Must have more than 5 years of experience in a Diabetes Education Unit Higher level diabetes management skills and with demonstrated ability for independent clinical decision making and autonomous practice, Must be an expert in the education of Diabetes Related Technology Leadership skills including highly developed skills in communication, problem solving, conflict resolution and negotiation skills, Please see the attached Role Description for additional information.
