About The Role
The NPC-Nurse Navigator is an advanced practice nurse that has the breadth and depth of clinical skills required to identify and monitor the health care requirements of palliative patients, and to provide support to identify the appropriate action required and to facilitate timely access to appropriate services. The NPC is required to be currently enrolled in, and working towards completion of, a Nursing and Midwifery Board of Australia (NBMA) approved Master of Nurse Practitioner program. NPC-Nurse navigators play a key role in supporting and coordinating a patient's entire health care journey, rather than focusing on just a specific disease or condition. This role is underpinned by the principles of delivering coordinated and patient-centred care, creating partnerships across different health providers and sectors, improving patient outcomes and enabling improvements across the system.
About You
Our ideal candidate will be someone who can demonstrate the following:
* Demonstrates advancing clinical evidence-based skill, knowledge and problem solving in the provision of specialist nursing consultation that optimises the clinical management of care for patients in palliative care.
* Demonstrates the ability to build and maintain strong relationships through open communication and the creation of a positive environment within and across the multidisciplinary team.
* Demonstrates the ability to educate, inform, counsel and support patients and families with contemporary clinical information at an advanced level relevant to patient's care and understanding.
* Demonstrated ability to interact and professionally partner with all members of the healthcare team both internally and externally to the health service.
* Demonstrated ability to look across the health system for improvements that will impact the care and service to older adults
Mandatory Requirements
* Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
* This position requires the incumbent to be enrolled in a Master of Nurse Practitioner course (or equivalent) and maintain passing grades during employment within the NPC Nurse Navigator position. Periodic requests for copies of academic transcripts may be required during employment within the NPC Nurse Navigator position.
