The Case Manager provides a collaborative approach engaging and supporting young people to successfully transition from care.
About the Role
The Case Manager works as part of a multidisciplinary team to address the impacts of abuse, neglect and separation on young people in out-of-home care and adults who have exited the care system.
* Assists in the recognition and de-escalation of behaviour using therapeutic crisis intervention techniques.
* Develops comprehensive plans for each young person to support their transition to independent living.
* Ensures young people are engaged with community supports.
