About the Role
We are seeking a Care Partner to join our Home Care Services team, supporting older people to live well at home across Regional South Australia.
This is a work-from-home role with regular travel to participants' homes and community settings. You will work closely with participants, families, care workers, clinical partners, and community organisations to design, coordinate, and review individualised care plans under the Support at Home Program.
This role is ideal for someone who is organised, compassionate, culturally responsive, and confident working independently.

Key Responsibilities
* Build strong, respectful relationships with participants, families, and carers
* Conduct culturally safe, strengths-based assessments
* Develop, implement, and review individualised care plans and quarterly budgets
* Coordinate services and communicate with aged care workers and providers
* Support participant choice, self-determination, and cultural identity
* Monitor changing needs, risks, and outcomes
* Ensure accurate documentation and compliance with legislation and standards
* Work collaboratively with Aboriginal Community Controlled Organisations and culturally specific services where appropriate

You are someone who:

* Can work autonomously while remaining part of a supportive team
* Builds trust easily with older people, families, and communities
* Is organised, proactive, and confident managing multiple participants
* Values continuous improvement and professional development

Essential Requirements
* Relevant experience in aged care, community services, or care coordination
* Understanding of (or willingness to develop) culturally safe practice when working with Aboriginal and Torres Strait Islander peoples and Elders
* Strong communication, assessment, and documentation skills
* Ability to manage budgets and care plans
* Current unrestricted SA Driver's Licence
* Reliable, roadworthy vehicle with full comprehensive insurance
* Right to work in Australia
* National Police Check (Supervised Contact with Vulnerable Groups)

Qualifications (One or more desirable)
* Certificate III or IV in Individual Support, Aged Care, Disability or Community Services
* Diploma of Community Services / Case Management / Ageing
* Diploma of Nursing (highly regarded)
