HARP Care Coordinator

We are seeking a qualified Allied Health Professional or Registered Nurse to join our dedicated HARP team. This temporary part time position is available from 25/09/2023-25/09/2024 and includes working Monday to Friday from This role involves care coordination for clients who have chronic and complex health conditions. The position requires assessment and assessment to our clients as well as working closely with a multi-disciplinary team.
Key Selection Criteria include:
Essential:
1. Registered Nurse Division 1 or relevant Allied Health Professional with current practising certificate from AHPRA
2. 2 years or more of continuous experience in acute or community health setting
3. Previous experience in rehabilitation programs
4. Advanced patient assessment skills
5. Advanced interpersonal, organisational, problem solving, time management and decision making skills
6. Demonstrate the ability to work effectively, both independently and as part of a multidisciplinary health team
7. High level computer skills to fulfil data processing requirements
8. Current drivers licence
Desirable:
9. Willingness to undertake further studies in Respiratory/Cardiac Management/Chronic Disease Management/ Rehabilitation
10. Previous experience in Care Coordination
11. An understanding of WHCG patient management systems
12. Motivational interviewing skills
13. Knowledge and experience in myagedcare
