Support Worker Opportunity

We are seeking a dedicated Support Worker to join our team and support vulnerable older people who require intensive assistance to navigate and access aged care services.


About the Role

The Support Worker will work closely with clients to identify their needs, connect them to appropriate aged care services, and provide tailored assistance to navigate aged care and other essential services. This role involves building trusted relationships, providing specialist and intensive assistance, and actively identifying and engaging with older people who are isolated or vulnerable.


Key Responsibilities

* Identify client needs and develop individualized plans to meet those needs
* Connect clients to relevant aged care services, including My Aged Care
* Provide tailored assistance to navigate aged care and other essential services
* Build trusted relationships with clients and their families
* Actively identify and engage with older people who are isolated or vulnerable


Required Skills and Qualifications

To be successful in this role, you will need:

* A tertiary qualification in a related field, such as aged care, community care, social work, allied health, or nursing
* Demonstrated high-level understanding of My Aged Care systems and processes
* Evidence of supporting older people, individuals with disabilities, or those experiencing social disadvantage
* Strong communication and administrative skills, including collecting, recording, and reporting data
* Experience working with case management software and other relevant digital tools
* Understanding of trauma-informed care and culturally appropriate practice
