Home Care Coordinator

Location: Edgeworth, NSW, 2258

Employment Type: Full-time

We're seeking a compassionate and proactive Home Care Coordinator to join our clients’ dedicated team and help ensure older adults and individuals with support needs can live independently and safely at home.


About the Role

As a Home Care Coordinator, you'll play a pivotal role in planning, coordinating, and monitoring care and support services for clients. Working closely with clients, families, and support staff, you'll ensure services are delivered with compassion, professionalism and efficiency.

Your day-to-day will include:

* Managing incoming referrals and conducting client assessments
* Developing individualized care plans tailored to client goals and needs
* Coordinating staff schedules and matching carers with clients
* Monitoring service delivery to maintain quality and client satisfaction
* Maintaining accurate client records and complying with relevant policies and procedures
* Responding promptly to incidents, feedback, or concerns
* On-call roster (1:4 typically) – allowances and call-out levy – additional $


What We’re Looking For

We’re after someone who combines warmth and empathy with sharp organisational and communication skills. To be successful in this role, you will need:

* Minimum Certificate III in Aged Care, Individual Support or equivalent (essential)
* Current First Aid Certificate
* Valid Australian Driver’s Licence
* Current Police Check
* NDIS Worker Screening Check
* Experience with home care packages
* Excellent interpersonal skills, with the ability to build strong relationships with clients, families, and staff
* Sound understanding of Aged Care Quality Standards and person-centered care principles
* Confidence using care management systems and Microsoft Office
* Strong time management and multitasking ability
