Therapeutic Case Manager Position

The ideal candidate will have a heart for trauma-informed, child-centred work, strong case management skills, and experience working with children, carers, and families in complex care environments.
As a Therapeutic Case Manager, you will provide specialist case management, co-create therapeutic care plans, lead transitions, and support foster carers managing high-needs cases.
Key Responsibilities:
* Provide specialist case management to vulnerable children and young people.
* Co-create therapeutic care plans that meet the unique needs of each client.
* Lead transitions and support foster carers managing high-needs cases.
Requirements:
* Tertiary qualification in Social Work, Psychology, Youth Work or similar field.
* Strong understanding of child protection, therapeutic models, and cultural competence.
* Ability to participate collaboratively within teams and work effectively independently.
