We are seeking a compassionate Social Worker to join our team at Belmont Aged Care home on a full-time, fixed-term basis; between January and June 2026, with the opportunity for permanency at the conclusion of the contract. This role provides maternity leave cover for our Allied Health team, working between residents in the aged care home and the Transitional Care Program and Time to Think program (located within the facility).
About the role
As a Social Worker, you will be responsible for undertaking comprehensive, goal-oriented and timely psychosocial assessments to understand the needs and circumstances of consumers. You will also be required to provide advocacy, case conferencing, crisis intervention, coordination of services, family support and education to enhance the consumers quality of life while living in our care.
You will be required to work across our Transition Care Program under the support and guidance of our current Social Worker, Transition Care Program Manager and Allied Health Manager.
You will have the opportunity to work closely with our Multi-disciplinary Team (MDT) including Occupational Therapists, Physiotherapists, Therapy Assistants and the residential and home care teams.
Your well-honed communication, assessment, and problem-solving skills are easily demonstrated when you work in a multidisciplinary team, as well as when making meaningful connections with consumers and their families.
What you'll bring
In order to be considered for this role, we need to ensure you possess:
* Tertiary qualification in Social Work (Bachelor or Master of Social Work)
* Australia Association of Social Work (AASW) membership (desirable)
* NDIS Worker Screening Check
* Influenza vaccination
* Full working rights in Australia.
* Knowledge on SAT and ACAT processes (desirable)
