The Opportunity - Benalla Location This role will work as part of a multi-disciplinarily team and in accordance with the objectives of the hospital, comprising of 0.4 FTE Dietetics and 0.6 FTE Intake Clinician. The successful candidate will work in Community Health Dietetics and back fill on the Acute ward and within Aged Care where appropriate. The position is permanent full-time. Key responsibilities include: Provide dietetic assessment and education for community health consumers presenting with a range of health and chronic disease needs. Delivery of consumer-centered care that meets best practice standards and is founded in an evidence-based approach. Maintain dietetic facilities in accordance with safety standards and infection control guidelines. Ensure monitoring of consumers and referrals to other service providers when appropriate to ensure broader health and well-being issues are supported. Demonstrate commitment to continual professional development. Act as a single point of intake for Community Health services and identify consumer needs and service priorities. Regularly liaise and provide information and advice to other agencies about services offered by Community Health. Process referrals and conduct initial need identification assessments for consumers referred to the service. Key selection criteria: Eligibility for full membership or provisional membership of Dietitians Australia and active Accredited Practicing Dietitian (APD) status High level organisational skills and ability to prioritise workloads and meet demanding work deadlines.
