About the Role As an Enrolled Nurse within Hampstead Rehabilitation Centre's Care of the Older Person and Community Transition (COACT) and Sub-Acute Geriatrics teams, you'll support older adults as they navigate important transitions in their health journey.
Working closely with Registered Nurses, you'll deliver person-centred care and help create a calm, respectful and supportive environment where patients feel safe, seen and valued.
Key Duties Deliver safe, person-centred care under RN guidance Assist with assessments and contribute to individualised care planning Monitor patient progress, recognise changes and escalate when needed Maintain accurate, timely documentation Build trusting relationships with patients, families and colleagues Collaborate effectively with health and community partners About You You're a compassionate and confident Enrolled Nurse who combines clinical skill with genuine respect for older adults and their families.
