The clinic operates seven days per week, with two GPs working on a rotating roster of 14 days on and 14 days off. They provide comprehensive primary healthcare services to a small mining town of approximately 1,300 residents, largely made up of young families and a significant industrial workforce. The clinic delivers a broad scope of care including general practice, chronic disease management, occupational health services, pre-employment medicals and preventative health programs. Their model integrates community-based primary care with industry-supported health services, creating a sustainable framework that supports both residents and the local workforce.
Position Description:
You are responsible for supporting patients with chronic and complex conditions under the Chronic Conditions Primary Care Package. Working within a multidisciplinary general practice team, the role focuses on care planning, patient education, coordination of referrals, follow-up and monitoring to improve health outcomes and reduce avoidable hospital presentations. The position involves proactive patient engagement, data tracking, liaison with external providers and contribution to quality improvement initiatives. You will ensure services are delivered in accordance with grant objectives, clinical governance standards and best-practice chronic disease management principles. They are seeking either Registered or Enrolled Nurse applicants. Accommodation is provided in Middlemount
