Our At Home Program includes: * Hospital in the Home (HITH) * Geriatric Evaluation Management (GEM) at Home * Residential In Reach (RIR) * Rehabilitation in the Home (RITH) Operating 7 days a week, the program provides a safe and effective alternative to hospital-based care through coordinated, multidisciplinary service delivery. Key Responsibilities As an At Home Nurse, you will: * Deliver expert, client-centred nursing care in home settings * Develop and implement individualised care plans * Advocate for clients and support their health goals * Provide education to clients and carers to promote independence and wellbeing * Monitor and respond to clinical deterioration and changing needs * Collaborate with multidisciplinary teams to ensure coordinated, high-quality care * Support transitions of care, including discharge planning and referrals to other services You will be actively involved across all phases of care, ensuring clients receive the right level of support at every stage.
