Job Description:
We are seeking a highly skilled Case Manager to join our Community Mental Health Treatment Team in Lismore. The successful candidate will provide case management within a multidisciplinary review team structure, delivering time-limited, evidence-based care to adults experiencing psychiatric illnesses.
The Case Manager will assess individual clinical loads, collaborate with a multidisciplinary treatment process, and manage patient outcomes using the principles of Trauma Informed Care and Recovery.
Requirements:
* Qualifications: Registered Nurse, Counsellor, Occupational Therapist, Social Worker, or Psychologist level 1/2
* Skills: Strong communication and interpersonal skills, ability to work independently and collaboratively as part of a multidisciplinary team
