About the Role
We are seeking an experienced Social Worker to join the team in a Rural Generalist capacity. This diverse role offers the opportunity to provide high-quality social work services across both hospital and community settings, supporting patients and families through a wide range of health and wellbeing needs.
As part of a multidisciplinary team, your responsibilities will include:
- Delivering social work assessments and interventions for hospital inpatients, including discharge planning, psychosocial support, and care coordination.
- Providing support across outpatient, emergency, and community-based settings to ensure continuity of care.
- Assisting patients and families to navigate health, social, and welfare systems, including aged care, disability, mental health, and child and family services.
- Promoting patient advocacy, wellbeing, and access to appropriate resources.
- Contributing to a collaborative team culture within a rural healthcare environment.

About You
You are a qualified Social Worker with a passion for delivering holistic, person-centred care. With experience across hospital and community settings, you are confident managing a varied caseload that includes inpatient, outpatient, and emergency presentations. You thrive in rural healthcare environments where adaptability, initiative, and strong problem-solving skills are essential.
You will bring:
- A recognised degree in Social Work and eligibility for membership with the AASW.
- Demonstrated experience in hospital-based social work, including discharge planning and psychosocial assessments.
- Strong knowledge of health, disability, aged care, and welfare systems, with the ability to advocate effectively for patients and families.
- Excellent communication and collaboration skills, with the ability to work effectively within a multidisciplinary team.
- A flexible, proactive approach to supporting individuals and communities in rural and remote areas.
