About you
We are looking for a compassionate and motivated Aboriginal Health Care Coordinator to join our team! You are passionate about improving health outcomes for Aboriginal and Torres Strait Islander people and thrive in culturally diverse environments. You are committed to delivering respectful, culturally safe, and high-quality care while supporting clients to navigate the health system and access the services they need.
You have strong communication, coordination, and program management skills, with the ability to oversee and support the delivery of community health programs that respond to local needs. You are confident coordinating care across services, monitoring program outcomes, and working collaboratively with multidisciplinary teams, community organisations, and families to ensure effective and culturally appropriate service delivery.
Your understanding of the social, cultural, and community factors that influence health allows you to build strong, trusting relationships and support holistic care for clients. Your passion for rural and remote primary healthcare, along with your ability to advocate for and empower clients, will make you an ideal fit for this role.
About the role
This role works within the clinical services team to support the planning, coordination, and delivery of community primary healthcare programs across the West Pilbara. The Aboriginal Health Care Coordinator will play a key role in improving access to culturally appropriate care by supporting clients to navigate the health system and coordinating services that address community health needs.
The role includes coordinating care for Aboriginal and Torres Strait Islander clients, supporting chronic disease management and preventative health initiatives, and assisting with the delivery of community outreach programs to towns and communities across the region. You will work closely with the clinical team to support program delivery, monitor outcomes, and contribute to health promotion activities that strengthen community wellbeing.
You will work in partnership with local Aboriginal Community Controlled Health Organisations, local aged care providers, and other community-based organisations to ensure coordinated, culturally safe, and holistic care for clients.
This role requires strong relationship-building skills, cultural understanding, and the ability to manage community health programs while supporting individuals and families to access the care and services they need.
