About the Role

We are looking to appoint a Grade 1 Social Worker, in a full-time (1.0 EFT) ongoing capacity, to work on the Monash Heart, Cardiothoracic and Ambulatory Care units at the Victorian Heart Hospital (VHH), part of Monash Health.

In this Social Work role, you will be responsible for triaging new referrals and meeting with VHH patients during their treatment onsite, or in the outpatient setting. The focus will be primarily conducting psychosocial assessments, risk assessments, providing emotional support and working collaboratively with relevant parties.

Having a sound understanding, and working knowledge of the range of the service systems including Family Violence, Child Protection, My Aged Care, Transition Care Program, and the National Disability Insurance Scheme (NDIS), Mental Health and the recovery model is essential, to ensure we provide high quality, evidence based social work service to all patients and families.

About Us

The Social Work Service at Monash Health assists patients and families with psychosocial support and advocacy through assessment, counselling, information provision, access to community resources, and discharge planning. Social Workers function as members of the multidisciplinary team and have a pivotal role where social complexity exists.

The Social Work Service operating hours are standard business hours Monday to Friday. A weekend service is provided by roster at Casey, Clayton, Dandenong and Victorian Heart Hospital sites and an on-call service is also provided to the acute sites.

About You

You are someone who is motivated to grow, be challenged, and make a meaningful impact, bringing enthusiasm for developing clinical expertise while contributing to high-quality, person-centred care.

With a strong interest in specialised clinical service delivery, you're confident undertaking complex psychosocial assessments and managing an active clinical caseload. Opportunities to expand leadership capabilities, assume broader clinical responsibilities, and contribute to project or research initiatives that enhance service delivery are genuinely appealing.

In a dynamic, supportive Social Work and Allied Health team, you collaborate effectively within a multidisciplinary model of care. Communication is one of your strengths, enabling you to build positive relationships with internal and external partners, and you maintain a strong commitment to evidence-based practice across both inpatient and community settings.

Qualification & Licence requirements include:

* Bachelor of Social Work or Masters Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW)
* Eligible for ordinary membership of AASW if overseas trained )
* A current Victorian Driver's Licence
