Our Housing & Homelessness Services deliver client‑centred, outcomes‑focused case management to individuals and families experiencing homelessness or at risk of homelessness, including women and children impacted by domestic and family violence.
About the role
Reporting to the Housing & Homelessness Coordinator, you will provide intensive, practical and flexible case management support to individuals and families with high or complex needs across Goulburn and Yass.
You will work collaboratively with clients and partner agencies to support safety, stability and pathways to long‑term housing.
Key responsibilities include:
* Conducting initial and ongoing comprehensive needs assessments
* Undertaking client risk assessments and developing safety plans, particularly in DFV contexts
* Developing and implementing individualised case plans in collaboration with clients and external services
* Advocating for and linking clients with housing, health, legal, DFV, mental health, AOD and financial counselling services
* Supporting pathways into long‑term housing, including private rental, community, social and affordable housing
* Providing practical assistance with housing applications, documentation, rental references and eligibility requirements
* Participating in a rotating on‑call roster (including weekends), as required
About you:
You are a motivated and organised case manager who enjoys working alongside people with complex needs and supporting meaningful change. You bring a trauma‑informed, strengths‑based approach and understand the impacts of domestic and family violence on housing instability.
You will have:
* A minimum Diploma in Community Services (or equivalent)
* At least two years' experience in a relevant case management role
* Sound knowledge of homelessness services, DFV, and referral pathways
* Strong communication, advocacy and documentation skills
* A warm, engaging and respectful approach with clients and colleagues
