Flintwood Disability Services is seeking an experienced Community Support Worker to play a vital role in providing assistance and support to individuals within the community. This position requires a compassionate and empathetic individual who can effectively communicate and build relationships with diverse populations. You will collaborate with various stakeholders to ensure the well-being and integration of individuals into the community.
Responsibilities
* Provide direct support to individuals in the community, including emotional support, advocacy, and crisis intervention
* Assist individuals in accessing community resources and services, such as healthcare, housing, employment, and education
* Develop and implement personalized support plans based on individual needs and goals
* Collaborate with families, caregivers, and other professionals to ensure a holistic approach to support
* Facilitate group activities and workshops to promote socialization and skill development
* Maintain accurate and confidential documentation of client interactions and progress
* Stay updated on relevant community resources, policies, and regulations to provide accurate information and guidance
* Participate in team meetings, training sessions, and professional development activities
Qualifications
* Certificate III or equivalent
* Proven experience working with diverse populations, including individuals with disabilities, mental health challenges, or other support needs
* Strong interpersonal and communication skills, with the ability to build rapport and establish trust
* Knowledge of community resources and services, and the ability to navigate and advocate within the system
* Ability to work independently and as part of a team, with excellent problem-solving and decision-making skills
* Proficient in using technology and software for documentation and communication purposes
* Valid driver's license and access to reliable transportation
