* Role Title: Social Worker - Inpatient Mental Health

About the Role:
* Permanent, part-time providing comprehensive social work services, including psychosocial assessments and risk evaluation, to patients, families, and carers in the Caritas Inpatient Mental Health Service.
* Develop and implement individualised treatment and care plans in collaboration with the multidisciplinary team.
* Lead safe discharge planning, coordinating referrals to community services and accommodation placements.
* Deliver counselling to individuals and families, supporting adaptation to medical and lifestyle changes, grief, and loss.
* Advocate for patients and carers with relevant government and NGO agencies, provide consultation and support, liaise with internal and external services, maintain accurate clinical documentation, and actively participate in multidisciplinary team activities.
About You:
* Degree in Social Work and eligible for membership of the Australian Association of Social Workers (AASW)
* Skilled in delivering comprehensive psychosocial care, including assessments, counselling, therapeutic interventions, case management, advocacy, and discharge planning — always with a person-centred and strengths-based approach.
* Specialised knowledge and hands-on experience in managing intensive mental health needs, supporting patients compassionately and effectively across both inpatient and outpatient settings with clinical acuity.
* Your an effective and empathetic communicator who excels in collaborative team environments, fostering strong, trusting relationships with patients, families, and multidisciplinary colleagues.
