About the Role

At the Lyell McEwin Health Service, social work is central to delivering care that goes beyond the clinical. As part of the Northern Adelaide Local Health Network (NALHN), you'll join a team committed to the health and wellbeing of one of South Australia's most diverse and growing communities — where your practice will make a tangible difference every single day.

Key responsibilities

* Conducting comprehensive bio-psychosocial assessments to inform care planning and discharge decisions
* Providing brief crisis counselling, advocacy, and direct support to patients and carers across acute and sub-acute settings
* Coordinating complex discharges in collaboration with consumers, carers, medical, nursing, and community-based services
* Contributing to NALHN's commitment to culturally & person-centred practice safe practice, particularly for Aboriginal and Torres Strait Islander patients

What You'll Bring

* A recognised finalised qualification in Social Work and eligibility for AASW membership
* Current eligibility to work in Australia
* AHP 2 a minimum of 3 years demonstrated tertiary hospital experience and supervising staff experience
* Ability to build on confidence & professional practice navigating complex, time-sensitive discharge planning processes
* Strong clear verbal & written communication skills for engaging with patients, families, and clinical teams
* An awareness and ability to have conversations about the management of Domestic & Family Violence, Child /Elder Protection, grief and loss including relevant legislations such as the Mental Health Act and Guardianship frameworks in South Australia
* Ability to work within teams, receive constructive feedback & effectively within SA Health's values of integrity, respect, and care

About You

In this role you will have strong interpersonal and verbal, written communication skills which will enable you to successfully problem solve, resolve conflicts and negotiate, including the ability to engage in difficult conversations, whilst building and maintaining relationships. An ability to effectively manage multiple, diverse and competing clinical priorities and demonstrated resilience within a stressful and complex environment is essential to this role. Must be prepared to work across a 7-day roster working some weekend and public holiday shifts.
