We seek an experienced professional at the Proficient or Advanced level, committed to delivering high-quality support to individuals with complex needs.
Key Responsibilities
1. Assisting clients and their families to identify needs through assessment and planning.
2. Developing, evaluating, and delivering evidence-based support plans in complex environments.
3. Creating meaningful goals and strategies using a positive support framework, aiming to eliminate restrictive practices in home and community settings.
4. Ensuring plans reflect clients' goals, passions, and strengths.
5. Building capacity and strengths of those supporting clients, including family and carers.
6. Collaborating within a multidisciplinary team, including mentoring less experienced professionals.
About You
* Proficiency or higher as a behaviour support practitioner with evidence of qualification.
* Significant experience supporting individuals with behaviours of concern.
* Excellent report writing, communication, and interpersonal skills.
* Valid driver's license and reliable vehicle.
* Genuine passion for the disability sector.
