- Aged Persons Mental Health Team**
The community teams have seven services located in two areas, in Kingston and the other is based in Endeavour Hills. The service provides assessment and treatment for people aged 65 years or over, who have a psychiatric disability and/or dementia with associated behaviours of concern requiring specialist treatment.
We currently Full Time & Part-Time opportunities with flexibility to work 1 day from.
**A Day in the Life**:
The Mental Health Clinician is responsible for a range of clinical mental health services including triage, intake, mental state examinations, risk assessments, care planning, treatment, case management and discharge planning.
The Mental Health Clinician will also act as a resource person to other staff in clinical problem solving and is also responsible for the promotion of Aged Persons Mental Health Services, supporting management in orientation and development of new staff and will carry a portfolio as designated by management.
As a member of a multidisciplinary team, you will also take part in a range of clinical mental health activities and have capacity to provide discipline specific services to clients as required.

You show a highly developed emotional intelligence within the care and support you provide. Understanding the importance of patient centred care, you have a genuine interest in working and interacting with our diverse teams to deliver high quality, safe patient centred-care.
Your critical thinking, willingness to learn and ability to work cohesively and share knowledge within a team will see you excel in this role whilst we provide you with guidance, support and training to learn the rhythm of the Aged Persons Mental Health Team's workflows.
You are also passionate about taking part in achieving excellence. Your calm and confident disposition will see you handle the needs of consumers as well as the needs of the teams whilst remaining focused on achieving exceptional outcomes. Your approach is patient centred and are confident working within a multidisciplinary teams.

You will also demonstrate:_**
- AHPRA registered OT or Qualified Social Workers (preferably accredited with AASW) or Grade 3 Registered Psychiatric Nurse
- Case Management or equivalent community mental health experience
- Aged Persons Mental Health experience highly regarded
