Clinical Care Coordinator & Nurse Mentor Role
Our organization is seeking an experienced and passionate Clinical Care Coordinator & Nurse Mentor to join our team. This full-time role reports to the General Manager, with support and guidance from the Clinical Care Manager while supervising and supporting our care team.
Key Responsibilities
* Provide clinical expertise and leadership of clinical and care service teams across the facility.
* Assist the Care Manager in leading carer teams to meet all clinical needs of residents – including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
* Manage all aspects of the assessment, documentation, and submission of AN-ACC requirements within the facility.
* Support the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
* Coordinate assessment, care planning, and review processes within the facility.
* Support the admission of new residents to facilities for optimal care assessments and initial consultations with residents and families.
* Ensure accurate information is recorded and in accordance with all organisational and legislative requirements (AN-ACC, resident records, and staff time recording).
* Input and oversight into the facility team rosters and ensuring optimal skill mix available for delivery of care to residents.
* Manage and document clinical risk within the facility and ensure a commitment to continuous improvements within the facility.
Desirable Qualities and Experience
* You will have an innate empathy for our residents and be committed to continually improving the quality of clinical care provided to them.
* Minimum 1 year experience as a Care Coordinator or similar leadership position.
* Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
* Post graduate qualifications in aged care would be advantageous but not essential.
* Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation.
* Engaging style that has the ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
* High level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
* A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes.
* Ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations that arise.
* Have a valid NDIS Workers Screening Clearance (or ability to apply via NDIS).
