**What You Will Be Doing**
Peninsula Health is committed to the implementation of Advance Care Planning (ACP) with a particular focus on adults with chronic complex disease, life-threatening illnesses and the frail aged. ACP facilitates an individual to appoint their medical treatment decision-maker, appointed support person and to document their care preferences in a legitimate format known as an Advance Care Directive or Instructional Directive. The Advance Care Planning Team Leader works as part of a team and is responsible for providing ACP line management, raising the profile of Advance Care Planning team and associated processes across Peninsula Health and engage with the community.
**What You Need**
We anticipate you will have:
- Current Vic drivers licence
- Be familiar with Victorian Legislative requirements
- Highly developed communication and interpersonal skills and demonstrated ability to work collaboratively and lead a multidisciplinary team
- Ability to operate steadily in an environment of change or experience in change management
- Previous leadership experience
