🧩 The Role
You will support NDIS participants to implement their plans and coordinate services that build capacity, independence and meaningful outcomes.
Responsibilities include:• Implementing NDIS plans and coordinating services• Supporting participants to connect with appropriate providers• Monitoring risks and ensuring supports remain effective• Building strong relationships with participants and stakeholders• Preparing professional documentation and reports for the NDIA• Supporting participants to build independence and capacity
👤 About You
We are seeking someone who:âś” Has experience working as a NDIS Support Coordinatorâś” Has a current participant caseload or established referral networkâś” Has strong knowledge of NDIS systems, reporting and complianceâś” Is organised and confident working independentlyâś” Is passionate about improving outcomes for people with disability
