The Clinical Nurse Specialist (Grade 2) plays a critical leadership role within the Care Navigation Care Coordination team, providing expert clinical oversight, specialist decision-making, and advanced coordination to support timely and safe patient transitions across the care continuum. This position guides frontline clinicians, leads complex care navigation, and supports the resolution of barriers that would otherwise delay discharge, elevate avoidable presentations, or extend patient length of stay. As the senior clinical anchor for the team, the CNS2 ensures consistent high‐quality assessment, facilitates prioritised workflows, and strengthens relationships with patients, primary care, and community services.
The role provides leadership capacity, enhancing the team's ability to deliver key service outcomes, and ensure that CNCC continues to provide safe, reliable and responsive care. This role directly contributes to improved patient experience, reduced system pressure, and overall service performance across P&CH.
Are You the Right Fit?
The ideal candidate is someone experienced with advanced clinical skills and a strong foundation in chronic and complex care management, supported by relevant postgraduate qualifications.
* You will bring at least three years of post‐registration experience and demonstrate the ability to exercise extended autonomy, critical thinking, and sound professional judgement in complex clinical situations.
* You will be confident mentoring and coaching nursing staff, contributing to quality improvement activities, and modelling person‐centred, culturally responsive care.
* Strong communication and interpersonal skills, proficiency in electronic health systems, and the ability to work both independently and collaboratively within multidisciplinary environments are essential.
Success in this role requires the capability to deliver comprehensive assessments, partner effectively with vulnerable clients and their families, and coordinate care across acute, primary, community, and social service providers. Selection Criteria

* Do you currently hold Authority to Practice as a Registered Nurse with AHPRA, relevant post‐registration qualification and at least 3 years full time equivalent experience working the clinical area of your qualification?
* Give a brief overview of what education training and clinically relevant experience you bring with you to the role?
* Demonstrated advanced clinical assessment skills and experience providing complex care to clients with chronic and/or complex health needs, including the ability to exercise extended autonomy and sound clinical judgement in a community based or integrated care environment.
* Proven ability to communicate effectively and engage with vulnerable clients, families, carers, and multidisciplinary healthcare teams to deliver safe, compassionate, culturally responsive and person centred care.
* Demonstrated capability to mentor, coach, and support nursing staff, and contribute to evidence based practice, quality improvement activities, and service development within a collaborative team environment.
* Demonstrated high-level critical thinking, problem-solving and organisational skills, including the ability to work independently, navigate complex care pathways, apply risk management principles, and utilise electronic health records and data systems effectively.
* Do you have a current Australian C-Class driver's licence (including P1 or P2)? if you hold a C class licence (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) licence within three months of appointment. Learner's licences are not permitted.
