Client Care Manager Role

We are seeking a skilled Client Care Manager to join our team. This role focuses on delivering high-quality care management services to individuals with varying disabilities and/or trauma.
The ideal candidate will possess exceptional communication skills, empathy, and the ability to coordinate complex care plans.
Key Responsibilities
* Provide expert care management advice and assistance to senior leaders.
* Oversee care management for clients with disabilities and/or trauma.
* Develop comprehensive care plans that meet individual needs.
* Conduct regular home visits to monitor client progress.
* Ensure timely incident reporting and compliance with relevant guidelines.
* Liaise with healthcare professionals to deliver personalized support.
* Manage safety and compliance across SIL housing, support workers, records, and documents.
* Complete monthly Client Needs Assessments.
Requirements
* Australian Citizen or Permanent Resident.
* Bachelor or Master's Degree in Human Services, Social Work, Special Education, or related field.
* Proven knowledge of Disability Care legislation, policy, and practice.
Other Requirements
* Driver Licence.
* Proof of Covid-19 Vaccination.
* Valid First Aid & CPR Certification.
* Working With Children Check (QLD Blue Card) and QLD NDIS Worker Screening Check.
