Community Nursing Role Overview
This community nursing position offers a unique opportunity to provide high-quality, person-centred care to clients in their homes and local settings. As a Registered Nurse, you will be responsible for delivering safe, effective nursing care, while building supportive relationships with families, General Practitioners (GPs), and other health professionals.
Each day begins with planning your schedule before heading out into the community to deliver wound care, medication administration, health assessments, chronic disease support, and health coaching. Using your clinical judgement, you will identify changes, elevate concerns, and adapt care plans as needed.
The role provides exposure to a wide range of clinical presentations, offering excellent opportunities for professional growth and development across community and chronic disease nursing.
* A motivated, adaptable Registered Nurse with Current Authority to Practice is required for this role.
* Excellent communication and time management skills are essential, as well as the ability to build meaningful relationships with clients.
* Demonstrated experience in wound management and/or relevant qualifications are highly desirable, along with confident clinical skills in assessment and chronic disease support.
