Job Description
This role involves providing comprehensive social work services, including psychosocial assessments and risk evaluations, to patients, families, and carers. You will be responsible for developing and implementing individualized treatment and care plans in collaboration with the multidisciplinary team.
You will lead safe discharge planning, coordinating referrals to community services and accommodation placements. Additionally, you will deliver counseling to individuals and families, supporting adaptation to medical and lifestyle changes, grief, and loss.
As a Mental Health Clinician, you will advocate for patients and carers with relevant government and NGO agencies. You will also provide consultation and support, liaise with internal and external services, maintain accurate clinical documentation, and actively participate in multidisciplinary team activities.
The ideal candidate will have a degree in Social Work and be eligible for membership of the Australian Association of Social Workers (AASW). They will thrive in suicide prevention settings, working side-by-side with Lived Experience Peer Workers to deliver fast, focused clinical assessment and care planning.
A strong commitment to trauma-informed, person-centered care that is culturally safe, non-clinical, and genuinely welcoming is essential. The successful candidate will be skilled in co-creating and reviewing care plans that reflect lived experience input and real-world needs.
