The Role
We are looking for an experienced CDM (Chronic Disease Management) Nurse, Enrolled or Registered, to join our Rockhampton Medical Centre. This role will support GPs and their patients in identifying and managing chronic conditions, aiming to keep patients out of hospital. The position is preferably full-time, but part-time options can be discussed with the right candidate.
The Tasks
In this role, you will be involved in tasks including, but not limited to:
1. Prepare Care Plans and Team Care Arrangements to help patients manage their diseases
2. Empower patients to take ownership of their health
3. Coach patients on health behavior change principles
4. Enable patients to access ongoing care from a multidisciplinary team
5. Assist in early identification of chronic disease through health assessments
6. Work with GPs to promote preventative healthcare
7. Increase the number of patients receiving a care plan for the first time
8. Ensure regular review of patient care plans
9. Manage administrative tasks related to CDM appointments

The Successful Candidate
About
We're seeking nurses passionate about patient care and collaborative team work. You will bring:
1. AHPRA registration as an Enrolled or Registered Nurse
2. Experience in Chronic Disease Management and health assessments
3. Knowledge of Health Assessment (highly regarded)
4. Strong organizational and time management skills
5. Excellent communication and customer service skills
6. A personable approach and passion for people
