About the Role
Support individuals living at home by assisting with daily activities, personal care, and community access, promoting independence and quality of life. Build meaningful relationships, support self-advocacy, and facilitate access to community services.
Key Selection Criteria
Essential:
* Certificate III or IV in Individual Support/Disability or equivalent
* Commitment to confidentiality and professional boundaries
* Experience working with people with disabilities of all ages
* Effective communication and active listening skills
* Ability to perform tasks safely and independently with vulnerable persons
* Relevant disability experience and knowledge
* Basic technical skills (email, internet, Smartphone, Microsoft Office)
* Good time management and communication skills
* Teamwork and person-centered care focus
* Experience in personal care provision
Mandatory Employment Requirements:
* Satisfactory National Police Check
* Satisfactory Working With Children Check
* Annual influenza vaccination (if applicable)
* Satisfactory NDIS Worker Screening Check
* Current Driver's Licence & Comprehensive Car Insurance
* Current Certificate II in First Aid
Key Responsibilities
* Assist with community activities and access, including accompaniment and transport support
* Provide personal care (bathing, toileting, dressing, grooming, mobility, transfers, medication monitoring)
* Support with domestic tasks and meal preparation
* Assist with shopping, bill payments, and errands
* Follow care plans and work towards participant goals
* Encourage confidence and independence in activities of choice
* Provide ongoing support, feedback, and participate in care planning
