We are seeking a Community Partnerships Officer to join our team. This is a permanent part-time position that will work across the Barwon catchment and the broader region.
The role will focus on building capacity within the community to address health inequities and improve health outcomes through collaborative action towards priority areas identified in the population health catchment plan.
Key Responsibilities:
* Develop and maintain partnerships with local communities, service providers, and stakeholders to advance population health priorities.
* Contribute to the development of the population health catchment plan and its implementation.
* Build capacity within the community to address health inequities and improve health outcomes.
* Collaborate with other teams and departments to leverage resources and expertise.
Requirements:
* Excellent skills and knowledge in health promotion, planning, implementation, and evaluation.
* Highly developed partnership development skills and experience.
* Demonstrated ability to work independently and as a team member.
