Based in Toowoomba, you'll deliver impactful case management support across both community and residential environments. Your work will empower individuals to secure stable housing, strengthen social connections, and build the confidence needed to maintain long-term independence.**Key Responsibilities**You'll champion high‐quality service delivery aligned with the National Homelessness Stream Model of Care, ensuring consistency and excellence across all client interactions.Your role will involve:* Building strong, trust-based relationships that foster self-determination* Delivering tailored, evidence-informed case management aligned with TSA best practice* Completing comprehensive assessments of needs, risks, and vulnerabilities* Continuously reviewing and adapting support throughout the client journey* Co-designing practical, achievable case plans that address barriers and build capacity* Implementing, monitoring, and adjusting case plans in partnership with clients**What You Will Bring*** A degree in Social Work, Community Services, Welfare, or a related field (preferred) **OR** extensive sector experience with demonstrated professional development* Proven capability in client assessment, case management, and social service delivery* A current QLD Working with Children Check (Blue Card)* Current First Aid Certificate (if designated as a first aider)* A valid QLD Driver's Licence**Why This Role Matters**This is your chance to make a meaningful, lasting impact—supporting individuals on their path to stability, independence, and a brighter future.
