The role:
Hands-on role supporting people with complex needs to live safely at home
We’re seeking a Team Facilitator (Senior Support Worker) to provide direct, recovery-focused support to people living with complex mental health conditions and intellectual disabilities.
You’ll work closely with individuals, families, and external stakeholders to ensure quality care and safe living at home.
What you'll do:
- Provide practical, social, and community-based support
- Assist with tenancy, appointments, and daily living
- Support customers with trauma, anxiety, drug use, or brain injury
- Liaise with families, health professionals, and other services
- Ensure support plans and documentation are accurate and up to date
What you need:
- Cert IV in Mental Health (not essential)
- Strong experience in mental health
- Ability to set boundaries while providing compassionate care
- Excellent communication skills and a proactive mindset
- Availability to work Monday to Friday 8am-5pm, (critical days: Tuesday & Friday
- NDIS Worker Clearance
- Valid Driver Licence with a reliable vehicle
