About the role
This rewarding opportunity will see you join the St Vincent's Private Hospital Toowoomba (SVPHT) as our Clinical Nurse Consultant - Discharge Coordinator. In this role you will ensure effective and timely admission/discharge processes are in place, case manage patients with an extended length of stay, and assist the Assistant Director of Clinical Services in developing strategies to maximise cost‑effectiveness and efficiency in the discharge process.
About the hospital
St Vincent's Private Hospital Toowoomba is the largest acute private hospital on the Darling Downs. We are a 197‑bed facility with highly experienced staff who work as a team to deliver the highest standard of healthcare. Our services include medical, surgical, maternity, paediatric, emergency, cardiac and rehabilitation.
Responsibilities
* Ensure effective and timely admission/discharge processes are in place.
* Case‑manage patients with an extended length of stay.
* Assist the Assistant Director of Clinical Services in contributing toward developing strategies to maximise cost‑effectiveness and efficiency in the discharge process.
* Consultative and liaison support with medical and nursing staff to enable the assessment, care, management and successful discharge of patients, coordinating with family and community organisations to ensure a smooth transition out of hospital.
Qualifications & Experience
* Registered with AHPRA (Australian Health Practitioner Regulation Agency) as a Registered Nurse.
* Ability to work on a permanent full‑time basis (38 hours per week).
* Previous experience in community and/or facility (or a similar role).
* Effective communication skills at all levels enabling best practice.
* Understanding of the importance of providing quality care and a desire to create genuine impact.
* Well‑developed interpersonal skills to support patients and organisational needs.
