Practice/Organisation Information: CWAATSICH's model of Care includes early intervention, engagement, as well as provision of Holistic Primary Health Care Services to our Aboriginal and or Torres Strait Islander communities throughout our region. The role will also provide care and coordination of the Nukal Murra Health Support Services - ITC (Integrated Team Care) Program funded through WQPHN. Position Description: Project support Work with the Care Access Manager to provide support in identifying and liaising with all GPs and allied stakeholders to be informed of the program including future ongoing updates. Identify staff to attend training in operation of the data base (portal) and ITC Guidelines and Procedures. Ensure that financial systems can track spending on ITC and invoice the NMHSS on a monthly basis; and Ensure clients are aware of the model and any future ongoing updates. Implement the ITC program in your region Liaise with the clients referring doctor/practice as required to confirm details/any specific requirements, for example, preferred service provider, where multiple services are involved for one client, seek advice from referring doctor/practice regarding order/timing of services/ appointments Engage general practitioners in your region Liaise with the clients referring doctor/practice as required to confirm details/any specific requirements, for example, preferred service provider, where multiple services are involved for one client, seek advice from referring doctor/practice regarding order/timing of services/ appointments. Check that there is no other funding source for each activity in Supplementary Services activity referral. Seek approval for each purchase from the Care Access Manager prior to purchase. Book/order and pay for approved ITC services for approved clients. Invoice NMHSS once monthly for all Supplementary Services expenditure against ITC. Engage providers/suppliers as required to complete each client request. Client liaison including: Communicate arrangements made e.g. equipment purchased, appointment date, transport/ accommodation details Provide practical support to assist clients to complete documentation relating to their referral, e.g., Referral forms in consultation with Practice Provide practical support to assist client to take up the services arranged. Liaise with referring doctor/practice to communicate client access issues as they arise, for example failure of client to take up the services arranged, logistical issues or changed arrangements. Liaise with referring doctor/practice to communicate outcome of referral, e.g. patient attended Liaise with providers to ensure report/documentation regarding the outcome of the service provided to the client is sent to the referring doctor to enable the doctor to provide post intervention follow-up. Program progress and issues Liaise with the Care Access Manager - MHSS and/or Practice Operations Manager in relation to progress of referrals and any issues/complaints that need resolving in your region; and Provide follow-up phone call to clients for feedback for Quality Assurance activities Data and reporting Complete data entry including client registration/ tracking/ outcome system; and Prepare reports to the Care Access Manager as required Self-Management& Teamwork Contribute to process improvement and adhere to all CWAATSICH Policies.
