**About the role
The Clinical Nurse Consultant (CNC) provides clinical expertise and leadership to the Residential Aged care facility Support Service (RASS) and the Geriatric Emergency Department Intervention (GEDI) in West Moreton Health (WMH).
The CNC RASS/GEDI provides an advanced practice role with a multidisciplinary approach to the screening, triage, assessment, care coordination and care planning of older people presenting to the Emergency Department (ED) or living within Residential Aged Care Facilities (RACF) in the WM catchment area. The role ensures the delivery of evidence based best practice older persons knowledge and skills, contemporary clinical leadership practice and management, to ensure safe practice and high-quality outcomes.
The RASS/GEDI team works collaboratively with older people, their caregivers, and the broader healthcare team to ensure the older person receives the appropriate care in the appropriate environment dependent on their individual care needs.
About you

Demonstrated ability to be an effective nursing professional with capability for high level engagement across the spectrum of stakeholders including patients and families.
Demonstrated contemporary clinical nursing expertise with clear understanding of the requirement for patient centred care.
Demonstrated legislation / policy knowledge and the ability to develop documents supporting the application of same.
Demonstrated effective communication skill including the ability to build and maintain effective relationships across disciplines.
Demonstrated management experience and capability with an ability and commitment to practice collaboratively within the service and the broader multidisciplinary health care team to deliver high quality care.
