Contract Role: Community Mental Health Worker
Job Description:
The Community Mental Health Worker will provide specialist services to a designated client group in the community setting, utilising evidence-based practices and recovery principles.
* Assess, support, and monitor the health status of individuals with Huntington's Disease and their carers, working collaboratively with primary health care professionals within an identified region.
* Deliver high-quality nursing care, partnering with patients/clients, their families, and other health professionals, to achieve safe, quality, clinically appropriate outcomes.
Required Skills and Qualifications:
* Registration with the Nursing and Midwifery Board of Australia as a Registered Nurse.
* Working with Vulnerable People registration (Tasmania).
* Desirable: previous experience working in a community setting.
