About This Role
We are seeking a Support Worker to support a wonderful NDIS Participant living with Cerebral Palsy who requires 24/7 care. This bright and vibrant Participant is ideally seeking a Support Worker who Speaks clear English and in addition FARSI his native language.
This role is perfect for someone who is passionate about helping others and delivering genuine, participant-focused support. The successful applicant will have the opportunity to make a real difference in the life of this remarkable individual.
Responsibilities:
* Provide high-level personal care including assistance with showering, toileting, dressing, grooming, feeding, lifting, and mobility support.
* Assist with fitting of aids and use of assistive devices as required.
* Support daily meal preparation, feeding, and monitoring dietary needs.
* Perform domestic duties including cleaning, laundry, shopping, and general household tasks.
* Offer social support and companionship both at home and in the community to promote social engagement.
* Provide transport assistance for appointments and community activities.
* Prompt or monitor medication intake in accordance with the care plan and company policy (training provided where applicable).
* Accurately document progress notes, clock in/out using our system, and comply with all reporting and policy requirements.
Requirements:
* Cert III or above in Disability, Individual Support or similar OR relevant experience.
* Experience in supporting NDIS Participants with personal care and community access.
* The Right to Work in Australia
* NDIS Workers Screening Check
* Working with Children Check
* All required NDIS Commision Training Modules
* First Aid and CPR
